So this is set in the original games' continuity, but has the Ultra protagonist as a cute kouhai for some reason?

I thought that girl at the end was the mom at first and I was like, "what, why would she want to steal her daughter away from Lillie?" That's when I finally realized that she's (probably) the USUM MC... that definitely makes a lot more sense.

This was nice. I especially like Lillie doing the "So cute I'm gonna die!" meme during her dream within a dream on page 4.

Also, I agree that the appearance of USUM!Mizuki was a bit confusing, especially since she was joking(?) that she'd steal away SM!Mizuki from Lillie if the latter took too long in Kanto. I wonder if this artist has drawn other doujins featuring the three of them before? At the very least, they've done fanart in the past.

Wat? So we started with a dream that she told Mizuki she was leaving and Mizuki confessed.

Then she woke up, realized she hadn't told Mizuki she was leaving.

Then the next morning Mizuki acts like she knew Lillie was going to Kanto. And USUM Mizuki is there for some reason??? EDIT: Wait, no, it's USUM Mizuki that knows about Kanto. I'm so confused.

And now Yuri NTR selfcest??????

But they were moving in for a kiss in the first scene--so why was Lillie all guilty and Mizuki saying that kiss she stole while Mizuki was asleep was her first?

EDIT: Oh, you're right on it not being a dream--on 8 she says right there "it's not a dream." Still doesn't totally explain the kiss and the random Kouhai!Mizuki but at least the order of events makes more sense.

Homura Subaru doesn't have any MizuLillie doujin. They made a clear file folder for comiket that one time, not an actual doujin.

Also, that Ultra Mizuki, I know that some people like to think about them as different characters (mostly as sisters), but it would be funny if in the Pokémon Adventures they really turn out to be different characters.
