There's really not a standard English for a blank, undefined word outside of a blank underline, and that would look silly in a title. As would (blank).

That explains why it's XX and not XXX. Yes. Should have made that connection.

English title maybe should only use one X? "do X" is a common phrase. Although it is somewhat perverted so "XX" is maybe fitting ^^

It's a pretty strange saying, but not terribly uncommon. I'd say it depends how long you've been into anime. People who have seen it for 4+ years tend to know it while those less don't. It's earlier if they're into manga or Visual Novels (IDK about LNs).
