If it's the latter, I'm wondering why a flashback wasn't shown about it. But then again, we haven't even seen much of Ran's life except when her father died.

It's most definitely about when they were their age. It could've been the latter if that comment was after they saw what the girl wrote. Even then it would be a pretty big stretch (heh) to make it about Ran having a boyfriend, and if it was the same situation (cheating on her) there would have been a flashback to it.
