"Not present ingame" doesn't equal "does not exist in the setting" seeing as how the former is a function of system and developement resources. It's not hard to discern from the lore that the geographical (and by extensions demographical) scales in-universe are actually considerably greater than the by necessity rather abbreviated gameworld, too.
That's the price to pay for a "seamless" openworld not split into clearly separated zones; in the latter the rest of the population and geography and whatnot is much simpler to handwave into the gaps. ¯\_(ツ)_/¯
