They seem to be both. Yes, this is a world where some Japanese school girls go around with paper bags on their head, while hiding behind pillars, and nobody notices them until another school girl comes along and helps them rip it off. At the same time, the entire thing seems to be a metaphor.

i thought that was the idea behind it. showing both the character in story tearing off the girl's paper bag and also confronting the reader. like the last few bubbles are talking directly at the reader, encouraging us to also do the same and help others out.
