Explanations for those curious:
The character on the right is the Plain Doll. She's a supernaturally animated mannequin that helps the player character throughout the game by converting experience earned into stat upgrades (she's a level up vending machine that occasionally comments on the player's progress in the story).
The character on the left is Maria, one of the bosses encountered towards the end of the game.
Neither of their backstories are ever explained in detail, and they never meet during the course of the game. There are bits of dialogue and text that allude to the Plain Doll being created in Maria's image.
Like many other games from the developer, exposition is sparse and characterizations are nebulous. If you're wondering what the context or deeper meaning of the image is, you know about as much as the artist themself.
