I'll make it simple: never ask if someone is "a girl or transsexual."

You can ask "are they a girl?", "are they a boy?", or better yet, "what gender are they?"

Then you can ask "are they transgendered?"

It's very important that you ask those as two separate questions, as doing otherwise can be extremely disrespectful.
