This seems like some sort of AU where Misaka is the leader of a gang (hence the many tattooes) and Kuroko (still in Judgment) is sent to arrest her, but failed quite miserably.

I think, in cases of selfcest where each version of a character has a name, there could be a couple tag for them (for example in this case, I don't know what are the names of the versions, but it could be Miku (V1) x Miku (Append)).

People never remember these two for whatever reason. I've even seen people say Nadeshiko and her unknown girlfriend are the only canon couple in the series.
