I prefer the interpretation that while Patche got it right with the "kisses don't make you pregnant" part, she is still totally ignorant about the reality of sexual reproduction thanks to having always asked Koakuma to fetch her books about that subject, and Koakuma deliberately avoiding the authentic sources in favor of completely misleading ones due to being either a troll or absurdly overprotective of Patche's innocence.
