I have never heard someone say or use this word and my girlfriend started laughing uncontrollably when I used this in a sentence. I asked "Sweetheart , do I ever do things that will make your heart penis rage to the point you can't take it anymore?" She woke from a dead sleep and just looked at me until she started laughing . Well now it's two hours later and she finally let me out of the handcuffs , and I don't think I can walk for the moment, thankfully I had my tablet on the nightstand ;) and I have learned that this word heart penis is not only a descriptive word but also a trigger setting of the moe button in your lovers head <3 So for now I need to crawl to the shower and somehow let my boss I will be late for work due to the fact that I can't stand or walk at the moment . actually you should say "Normally Thai women not smoke" than that sentence.
It not like smoking is banned or people will treat you different or anything.
It just from the point of view that other women didn't do this thing which is considering as a bad thing ,people will have a strong felling (both good and bad) to the one who do it . anyway it the same for men but people will have a softer felling that all.
