I feel like the hang-up on Bocchi not being Kita's first is less a "purity" thing, and more that Bocchi is incredibly insecure in their current relationship.

I'm pretty sure that's the right interpretation. Bocchi's thought "Wouldn't it being nice if I could have been your frist too" came only after Kita told her "Your first always would be me. Only me, everlong." Kita was telling her that no matter how many people she meets or how closer she get with them, there's a place that only Kita have (the person who took Bocchi's first time) and nobody else can take that place, nobody else can have that closeness with Bocchi.

Whatever if they're friends with benefits or a couple (for what is show, I think both are possible) is clear that both have romantic feelings for each other. Kita's jealousy towards the girl who aproached Bocchi clearly shows she sees Bocchi as more than a sexual partner (there's no reason to be jealousy for fans or people getting closer to her if she only care about her body), and the lyrics of the song (everlong) playing while they're having sex talks about someone who wants to keep the closeness they have with another person forever but is afraid because they think that are losing it or can lose it. I think that is describing the feelings both, Bocchi and Kita have about their relationship.

From Kita's side, she at least has the fact that she took Bocchi's first (and if they're in fact dating, also the place of first girlfriend) as an insurance that no matters what happens in the future, she has a special place in Bocchi's memories/heart that nobody can clain now, a unique conection with her. Bocchi on the other side has nothing. From her view point at least, Kita already had another partner/s (romantic, sexual, both, it doesn't matter), so, for Kita there's nothing that difference her from "the past one/s". That is what makes her regreting not being "Kita's first", the fact that she has no significant thing to tie herself with Kita, which prabably makes her think that for Kita she's another "temporary partner" that she can drop whatever she gets bored of her (because again, that's what Bocchi thiks popular girls do).

At the end, this is a story about two people who really love each other and want to be together forever but are insecure about having a future togeter, while at the same time being unable to share their feelings/worries with their partner.

Because none of those looks down meant something in long term. In the end of the day, Kita is still closer with Bocchi than with anyone else, and vice versa.
Plus, in the latest chapters has also been proven that Bocchi is as gay like Kita. That also hepls.
