Uh I think
I think that, since one's patronus reflect one's heart, and her patronus is actually the same as Eli's (it's a fox right), that it's supposed to mean that Eli is the one in her heart

I think this is like how Snape has the same patronus as Lily Potter, it shows Snape's love for her even after all this time.

ooohh i think i saw an explanation from tumblr about this
remember when eli said that it reflects a person's heart? yeah, well it was said that someone's pratonus will be similar with the one they love (if deep, deep love and affection). and since nozomi loves eli, her heart is full of eli, making her pratonus similar to eli's.
(also explains why snape has the same patronus as lily)

Not necessarily, James and Lily Potter had different patronus (James' was a stag, and Lily a doe; rather, it's Harry who had the same patronus as his father.)

Hi! So as the original translator (and a huge HP nerd), here's the explanation: Your Patronus can change to match (so they don't have to be the same necessarily) the Patronus of the person you love, but it will only do so if that love is eternal, unchanging, and part of you forever (e.g. it will only match your one true love, and won’t change for a person who you’ll eventually fall out of love with; this "eternal etc." was explicitly said in a tweet by JKR btw).
And that’s why Nozomi can’t show Eli her Patronus - because Eli would realize that Nozomi is in love with her. IIRC the original artist on Twitter said something about how this was part of an AU scenario in their head where Nozomi suddenly realized that she was in love with Eli, and that shock caused Nozomi's Patronus to shift into the same fox form as Eli's.
As for the James/Lily thing, gendered versions of Patronuses exist (e.g. stag/doe, lion/lioness, stallion/mare), so couples can have complementary (rather than identical) Patronuses since the charm is cast using happy thoughts, and your loved one is usually the basis for those.

Another thing with the not matching patronus: Ron and Hermione’s patronus’ were a Jack Russell terrier and an otter respectively. Jack Russell’s are known for hunting otters. So in a sense they did have matching patronus because theirs were opposites of each other. If you love someone deeply and true, your patronus can end up matching like that. If you love someone as an equal then yours are gendered opposites. If you love one but struggle to express it then they can be enemies. There is also just not match but still love. Ginny’s was a horse and Harry’s was a stag.
