This manga might be Morinaga Milk's weirdest work. I don't understand why the deeply concerning health issues aren't being addressed, treated seriously, or anything. They're kind of there to just make everything feel weird and awful.

My guess is because of how bad the current condition for psychological help system in Japan is, Rena really just doesn't understand how much Yuna really needs help. She sees the sleeping problem but doesn't grasp how deep it goes or what she can do about it (hence we see her reading up about it but not yet finding something that clears things up a bit more for her).
Rena seeing Yuna work around her sleeping problem is probably making it hard for Rena to bring up the doctor. It's definitely not helping that these two have trouble communicating.

It's not a 1 to 1 thing but that doujin exists. Utena is a most of the girls have their roles switched-ish but Utena is still Utena even as a magical girl. And it has multiple chapters and with more comming.

It's still up in the air for me who will confess, but I'm actually leaning toward Miyagi with the way things are going now. Or we might see a kind of scene where Miyagi confronts Sendai with the question, straight up asking "Do you want us to be lovers?" and then Sendai will have to answer honestly. I could definitely see a scene where Miyagi asks this, and Sendai returns the question with a "What do you want, Miyagi?" and that causing a big fight.

While I agree that a confession from Miyagi is far more likely. Her asking to be lovers while giving that necklace this early(???) on is simply too hard to imagine. She's still at the stage where she doesn't fully understand and has yet to process that she likes Sendai in that way.
