I think its because of the way Reina's face was drawn that's throwing people off.. its just the art style this is a Fan Art

I always assumed Reina wore the pants ? although she admitted "she likes pain" and kumiko said "that was hot" so i reckon instead of rubbing antiseptic she would just lick it cos it would sting more or give pleasure? lol
