^Ochako had Himiko laying on her hand for a night so it gets numb. And now you can guess what Himiko fingers have been doing for a long period of time here. Notice wetness on Ochako's shirt and Himiko's hand.

The way I'm reading it is that Toga broke in while Ochako was asleep, diddled herself crazy, and fell asleep in the act (fingers still inserted).
And yeah, arm pillows are nice but the deadarm afterwards is kinda not worth it.
