I'm gonna ruin it by saying that this is portraying her last day, Akyuu knows that her time has come so she wants her final moments to be with a close friend sharing a nice and comfortable moment, maybe she requested Akyuu to read her something like a fairytale so she can drift away to the sound of her voice, and Kosuzu is oblivious about it, thinking that Akyuu is just more chummy than usual.

Her second tail is hidden by her first tail behind her back so Orin looks like a ordinary cat waiting for food.

If it was indeed Akyuu's last day I doubt she would be allowed to even leave her home, similar to how in The Gensokyo of Humans, as soon as the Hieda suspects that Akyuu could maybe perhaps by chance be close to dying they immediately shut her off the world to prepare for her reincarnation.
