She died from mental shock of going through puberty. She complained her father didn't see the real her. She didn't want to grow up. She changed her name from masculine one, to feminine one. She was happy to become zombie, because she won't ever change this way. Saki said "whatever [she] has something attached or not, it doesn't matter" referring to penis. Both original japanese and crunchyroll translation were equally explicit about it. Only people who don't want to believe it and/or deny it, are transphobic people.
