The only reason I can think of for the genderswapped joker being so popular is the song in Dancing Star Night and the opening to Persona Q 2, where he inexplicably starts singing in a woman's voice (well... he's animated singing in a part that's sung by a woman, which is pretty close).

First instance of Lady Joker appearing on this site was an early version of that Werewolf x High School Girl doujin that popped up only a couple months after P5 released globally and DSN + Q2 weren't announced yet.
Even before that Lady Joker was already a staple in Persona fanworks due to his purposefully androgynous appearance (while still somehow being hella baller manly at the same time? imho) and the art book containing concept art of a female Joker, since they also planned to have selectable genders (for the 3rd time in a row.)
As much as I hate to kill a joke, I'm pretty sure it's all just a rather big coincidence. The adorable shy scrawny meganeko-type has been a staple trope for a long time now. Ami Mizuno, anyone?
