On the matter of the boobs - they are not just meant to be 4 x bigger than Tooru's in the show, I'm fairly sure the artist has just messed up perspective and there are 2 boobs there. The previous Kobayashi X Tooru image is by the same artist - and has reasonable boob size.

Both legs need fixing too.. I might edit this for myself for a desktop - It's a nice picture if you fix this issues.

Also - I almost missed Kanna & Saikawa in the doorway lol. I wonder if they will use what they learn? :)

Not any movie in particular. Smoking in bed is a common convention in media (American in my experience, but it may or may not apply in other cultures) frequently used to convey to the audience that sex has occurred without explicitly showing the act taking place.

The act of smoking tends to imply the smoker is sexually experienced, which is then contrasted with the inexperience of their partner, as demonstrated in the above image.

Smoking after sex is a classic meme. It is used as memetic shorthand as a way of confirming that, yes, they definitely just had sex. Not only that, they literally just finished up a few moments ago.
